What this site collects
The enquiry form collects your name, email, phone, country, care area, language, provider preference and optional message. Please do not include clinical records or passport information.
Why we use it
Your enquiry is used to respond and discuss care coordination. Separate consent should be obtained before sharing medical records or identifiable information with a provider.
How enquiries are handled
Enquiries are emailed to umazahmt@gmail.com through the hosting server. Email is not a secure medical-record portal. Delivery depends on hosting mail configuration.
Storage and external services
Theme and motion preferences are stored in your browser. There are no analytics or advertising cookies. A server session protects form submissions. Rate limiting uses a temporary hashed network identifier. Language assistance opens Google Translate only after your action; its privacy practices apply. WhatsApp and Instagram have their own policies.
Your choices
For access, correction or deletion of enquiry information, email umazahmt@gmail.com. The operator must review retention needs, keep enquiries only as long as needed and publish its final retention schedule before launch.
